Detox centers in Massachusetts

72 Massachusetts programs in our directory provide detox, the medically supervised first step of treatment: 54 on an outpatient basis, 14 in a residential setting and 10 inside a hospital. Some programs offer more than one setting, so the three numbers overlap.

Detox programs by setting in Massachusetts: Outpatient detox 54, Residential detox 14, Hospital inpatient detox 10, out of 72 detox programs.

Inpatient detox in Massachusetts: what the state calls ATS

Massachusetts agencies use their own names for the first steps of care, so a program's website may never use the word detox. The Bureau of Substance Addiction Services describes three levels that usually follow one another:

  • Acute Treatment Services (ATS). Withdrawal management with nursing and medical supervision 24 hours a day, seven days a week, in a freestanding or hospital-based setting. This is inpatient detox.
  • Clinical Stabilization Services (CSS). Round-the-clock supervision, observation and support in a nonmedical setting. It is usually the step right after ATS.
  • Transitional Support Services (TSS). Short-term residential support for people who need a safe and structured place between detox and residential rehab or outpatient care.

These are the terms used on state pages and by licensed programs, so it helps to know them before you start calling.

Outpatient detox programs

The state's term is outpatient withdrawal management: clinical management of withdrawal symptoms with FDA-approved medications in an ambulatory setting, sometimes with counseling. 54 Massachusetts programs report outpatient detox. For alcohol, the National Library of Medicine says people with milder symptoms can often be treated in an outpatient setting as long as someone stays with them, while moderate to severe withdrawal may need a hospital or another inpatient facility.

Alcohol detox and drug detox in Massachusetts

Not every detox program treats every substance. Of the 72 Massachusetts detox programs, all report opioid detox, 60 methamphetamine detox, 59 cocaine detox, 37 alcohol detox and 36 benzodiazepine detox. That leaves 35 detox programs that do not list alcohol, so check before you call. The Alcohol detox filter shows the ones that do.

Detox programs by substance in Massachusetts: Opioid detox 72, Methamphetamine detox 60, Cocaine detox 59, Alcohol detox 37, Benzodiazepine detox 36, out of 72 detox programs.

Alcohol deserves particular care. The National Library of Medicine notes that alcohol withdrawal can range from a mild disorder to a serious, life-threatening condition, that symptoms tend to start within 8 hours of the last drink, and that people with moderate to severe symptoms may need treatment in a hospital or another facility that treats withdrawal. That is the case for choosing a drug and alcohol detox center over stopping alone. Our guide to medical detox explains what the first days usually involve.

Detox programs that take MassHealth

62 of the 72 detox programs report accepting Medicaid, the program Massachusetts runs under the name MassHealth. Show detox programs that take MassHealth in the list above.

How to compare detox facilities

There is no official ranking of the best detox in Massachusetts, and we do not publish one. What you can check is concrete: whether the setting is inpatient or outpatient, whether a nurse or physician is on site around the clock, which withdrawal medications are used, and where the program sends people after detox ends.

Inpatient and residential rehab in Massachusetts

Inpatient rehab in Massachusetts mostly means residential care: 105 programs offer residential rehab, where people live at the facility during treatment. 46 describe themselves as long-term and 23 as short-term. A further 30 provide hospital inpatient care.

Residential and inpatient programs in Massachusetts: Residential, any length 105, Long-term residential 46, Hospital inpatient 30, Short-term residential 23, out of 406 programs with a SAMHSA service profile.

Inpatient alcohol rehab and inpatient drug rehab

If you are looking for inpatient alcohol rehab in Massachusetts, or for inpatient drug rehab, the pool is the same 105 residential programs. SAMHSA profiles describe residential programs by setting and length of stay, not by substance, and there is no separate federal list for either. If you are weighing a live-in program against treatment from home, see inpatient vs outpatient rehab and our overview of inpatient rehab.

Long-term and short-term residential programs

Residential rehab in Massachusetts is not one thing. The Bureau of Substance Addiction Services licenses several models, all staffed 24 hours a day:

  • Recovery homes offer a structured, sober environment and encourage residents to use community resources.
  • Therapeutic communities are highly structured, and residents take an active role in their own treatment.
  • Social model programs center on sober living, peer counseling and case management, with residents working or studying outside.
  • Co-occurring enhanced programs are for people recovering from addiction and a moderate to severe mental health condition.

Ask which model a program follows before you compare lengths of stay.

Outpatient rehab in Massachusetts

314 Massachusetts programs offer outpatient rehab, which makes it the most widely available kind of care in the state. 92 run an intensive outpatient program (IOP) and 95 offer day treatment or partial hospitalization.

Counseling approaches and telehealth in Massachusetts: Cognitive behavioral therapy 367, Telehealth 321, Motivational interviewing 205, Relapse prevention 202, Trauma-related counseling 179, out of 406 programs with a SAMHSA service profile.

Outpatient drug rehab and counseling

Outpatient drug rehab in Massachusetts means treatment you attend while living at home. Among profiled programs, 367 list cognitive behavioral therapy, 205 motivational interviewing and 202 relapse prevention. 321 offer telehealth sessions. More detail is in our guide to outpatient treatment.

IOP rehab in Massachusetts and day treatment

Massachusetts insurance guidance defines an intensive outpatient program as structured treatment for two to three hours a day on multiple days a week, and partial hospitalization as day or evening programming available five to seven days a week. Our pages on the intensive outpatient program and partial hospitalization, also called day treatment, compare the two.

To see them, use the Outpatient or IOP filter in the list above.

Alcohol rehab in Massachusetts

132 Massachusetts programs administer or prescribe medication for alcohol use disorder, according to their SAMHSA profiles, and 37 provide alcohol detox.

Alcohol rehab centers and programs

Alcohol rehab in Massachusetts is delivered by the same network of drug and alcohol rehab facilities described on this page. For people with moderate to severe withdrawal symptoms an alcohol rehab program begins in a detox setting, described above, and continues as residential or outpatient care. Our page on alcohol rehab walks through the stages.

Medication for alcohol use disorder

The National Institute on Alcohol Abuse and Alcoholism states that three medications are currently approved in the United States to help people stop or reduce drinking: naltrexone, acamprosate and disulfiram. In Massachusetts, 136 profiled programs list oral naltrexone, 109 disulfiram and 104 acamprosate.

Medication for alcohol use disorder in Massachusetts: Naltrexone (oral) 136, Disulfiram 109, Acamprosate 104, out of 406 programs with a SAMHSA service profile.

Drug rehab in Massachusetts: opioid, cocaine and medication treatment

134 Massachusetts drug rehab programs prescribe buprenorphine, 127 prescribe naltrexone for opioid use disorder and 36 provide methadone maintenance. 39 are federally certified opioid treatment programs.

Medication treatment offered in Massachusetts: Buprenorphine 134, Alcohol use disorder medication 132, Naltrexone 127, Methadone maintenance 36, out of 406 programs with a SAMHSA service profile.

Methadone clinics in Massachusetts

Methadone for opioid use disorder cannot be handed out by an ordinary clinic. The state is explicit that only federally certified, accredited and licensed Opioid Treatment Programs may dispense methadone. An opioid treatment program, or OTP, is certified by SAMHSA and licensed by the Bureau of Substance Addiction Services. That is why the list of methadone programs is much shorter than the list for buprenorphine.

The Department of Public Health has studied how far people have to travel to reach one. It has published geographic access studies of opioid treatment programs for Boston, Worcester and western Massachusetts that identify where new programs should be placed, alongside its overdose reports.

Buprenorphine and naltrexone

Buprenorphine, the medication in Suboxone, and naltrexone are prescribed in office and clinic settings as well as in opioid treatment programs. 125 programs in the state run outpatient medication treatment. Our guides to opioid treatment and medication-assisted treatment explain how the three medications differ. Show programs that prescribe buprenorphine.

Cocaine and other drugs

For cocaine, methamphetamine and other drugs, the profiles show which counseling approaches a drug rehab program uses. The ones reported most often here are cognitive behavioral therapy (367 programs), motivational interviewing (205) and trauma-related counseling (179). 59 programs offer cocaine detox in Massachusetts and 60 methamphetamine detox. See cocaine addiction treatment for what a typical plan looks like.

Paying for rehab in Massachusetts: MassHealth, BCBS and other insurance

345 of 406 profiled Massachusetts programs (85%) accept Medicaid. 351 take private health insurance, 270 Medicare and 205 TRICARE, and 104 use a sliding fee scale.

What programs accept in Massachusetts: Private insurance 351, Cash or self-pay 351, Medicaid 345, Medicare 270, TRICARE 205, Sliding fee scale 104, out of 406 programs with a SAMHSA service profile.

MassHealth

In Massachusetts, Medicaid and the Children's Health Insurance Program are combined into one program called MassHealth. SAMHSA's data says Medicaid, so the 345 programs counted above are the ones to call first if you are a MassHealth member. What is covered depends on your MassHealth plan and on the level of care, so confirm it with the program's admissions staff before you go. The MassHealth (Medicaid) filter in the list shows every program that reports accepting it.

BCBS and other private insurance

351 programs report accepting private health insurance. The federal profile does not say which insurers a program is in network with, so a plan such as Blue Cross Blue Shield of Massachusetts has to be checked program by program. Our page on Blue Cross Blue Shield rehab coverage lists the questions to ask. The network question is the same for a residential stay and for BCBS Massachusetts outpatient rehab.

State law limits how an insurer can delay the first days of treatment. Under Chapter 258 of the Acts of 2014, insured health plans regulated by Massachusetts must cover medically necessary acute treatment and clinical stabilization services for at least 14 consecutive days without preauthorization, provided the facility notifies the insurer within 48 hours of admission. The Division of Insurance set this out in Bulletin 2015-05, issued July 31, 2015. The bulletin defines an insured plan as one where a licensed carrier takes on the risk in return for premiums. Ask your plan whether it is insured in that sense or self-funded by an employer, because the rule is written for the first kind.

No insurance: free and state-funded treatment

104 programs use a sliding fee scale based on income and 73 report other payment assistance. The Bureau of Substance Addiction Services lists providing access to treatment for the uninsured among its responsibilities. Start with the state Helpline at 800-327-5050, described below, and say that you have no insurance.

For how plans handle rehab in general, see our insurance coverage guides.

Rehab for women, men, teens and veterans in Massachusetts

112 Massachusetts programs run a track for adult women and 123 for adult men. 173 accept children or adolescents, 147 have programming for veterans and 99 serve pregnant or postpartum women.

Programs for specific groups in Massachusetts: Children and adolescents (accepted) 173, Young adults (dedicated program) 173, Veterans 147, Adult men 123, Adult women 112, Pregnant or postpartum 99, out of 406 programs with a SAMHSA service profile.

Women's rehab and men's rehab

Women's rehab in Massachusetts usually means a dedicated track inside a mixed program, not a single-sex facility: only 12 programs admit women exclusively. The same holds for men's rehab in Massachusetts, where 25 programs admit only men. Read more about women's rehab and men's rehab.

Massachusetts also funds residential programs built for particular groups. The Bureau of Substance Addiction Services describes Specialized Residential Services for Women as programs where women can receive residential treatment while keeping custody and care of their children, and Residential Rehabilitation for Adolescents and Transition Age Youth as short-term residential care for people under 26. There are separate family programs for homeless families in which a parent has a chronic substance addiction.

Teens, young adults and veterans

Accepting adolescents is not the same as having a program built for them: 44 programs list a dedicated adolescent program and 173 one for young adults. Families looking for adolescent care can start with teen rehab, and former service members with veterans rehab. The Teens and Veterans filters show the programs.

Spanish and sign language services

109 Massachusetts programs report providing services in Spanish and 152 offer sign language services for people who are deaf or hard of hearing. The profile does not say how many staff speak a language, so ask when you call.

Mental health, depression and trauma: dual-diagnosis rehab in Massachusetts

347 Massachusetts programs treat a substance use disorder and a mental health condition together, which clinicians call co-occurring or dual-diagnosis care. 196 programs provide mental health treatment in its own right.

Mental health and trauma services in Massachusetts: Co-occurring disorders 347, Trauma services 224, Mental health treatment 196, PTSD services 99, out of 406 programs with a SAMHSA service profile.

If depression, anxiety or trauma is part of the picture, look for mental health rehab in Massachusetts that also treats substance use. The SAMHSA profile has no separate category for depression rehab in Massachusetts; the closest is co-occurring care, where one team treats both conditions. 224 programs here report services for people who have experienced trauma and 99 for post-traumatic stress disorder. Our guide to dual diagnosis treatment covers what integrated care includes. Show dual-diagnosis programs.

For residential care, Massachusetts licenses co-occurring enhanced programs that support recovery from addiction and a moderate to severe mental health condition at the same time. When the main risk comes from a mental illness rather than from substance use, the state's own guidance says Section 12 may be more appropriate than Section 35. Both laws are explained under state resources.

What "best rehab in Massachusetts" means and how to compare programs

No agency ranks the best rehab in Massachusetts, and this directory does not rank programs either. The list on this page is in alphabetical order by city, so a program's position says nothing about its quality. What can be compared is what a program is licensed and accredited to do.

  • State license. 146 profiled programs report a license from the state substance use treatment agency.
  • Accreditation. 91 are accredited by The Joint Commission and 62 by CARF.
  • Ownership. 199 are run by private non-profit organizations and 203 by private for-profit companies.
  • Level of care. The best drug rehab for one person is the one that offers the setting they need, whether that is detox, residential or outpatient.
  • Payment. Confirm your plan with admissions before arrival, not after.
  • Aftercare. Ask where people go when the program ends and who arranges it.
License, accreditation and ownership in Massachusetts: Private for-profit 203, Private non-profit 199, State agency license 146, Joint Commission 91, CARF 62, out of 406 programs with a SAMHSA service profile.

Understanding treatment in Massachusetts

Three things shape whether a person in Massachusetts can access treatment: where they live in the state, what insurance they carry, and which clinician answers the first call. The 589 licensed facilities do not change that calculus; they constrain the choices within it.

The Medicaid question

The Medicaid story in Massachusetts: Massachusetts expanded Medicaid in 2014 under the Affordable Care Act. Has realistic access to Medicaid coverage for addiction treatment once enrolled. No individual clinical decision, no facility-level quality variation, changes the underlying math. States that expanded have a treatment system; states that did not have a triage system.

The overdose-mortality context

The overdose rate in Massachusetts is 32.8 per 100,000 — a number that is rarely cited without caveat, because averages smooth out the specific places and specific populations where death concentrates. integrated state-funded treatment system strains under high demand That geographic and demographic inequality is the thing the top-line number cannot tell you.

How access actually works in Massachusetts

Inside the 589 licensed facilities in Massachusetts, the clinical quality variation is substantial. The practical context here is that integrated state-funded treatment system strains under high demand — which is why the difference between a well-run IOP and a fee-for-service residential chain that churns patients through 30-day cycles is not visible from the outside. It becomes visible when you ask the specific question: "Does this program offer buprenorphine for opioid use disorder?"

What to do next

Practically, three things happen next if someone in Massachusetts is going to get help: a clinical assessment (by someone whose incentives are clinical, not financial), an insurance verification (in writing), and a facility selection (ASAM-aligned and MAT-inclusive). In that order. Reversing the order is the most common source of the "they said they took my insurance but I got a $15,000 bill" stories.

Last updated October 2026. Sources: SAMHSA Treatment Locator, CDC WONDER (overdose mortality 2023), KFF Medicaid Tracker, ASAM Criteria 4e. See our editorial policy.

Massachusetts law and state resources

Massachusetts Substance Use Helpline

The Massachusetts Substance Use Helpline is a statewide public resource for finding treatment, harm reduction and recovery services. Call 800-327-5050 or text HOPE to the same number. It is free, confidential and open 24 hours a day, every day of the year including holidays. The Helpline's website is also available in Spanish.

Section 35: court-ordered treatment

If you are reading about Section 35, you are probably worried about someone's safety, and that is a sound reason to learn the rules. Section 35 of Chapter 123 of the Massachusetts General Laws allows a qualified person to ask a court to order involuntary treatment for an alcohol or substance use disorder. The state's own guidance says involuntary commitment should be the last option for treatment, because people who take part in the decision are more receptive to care.

  • Who can petition: a police officer, physician, spouse, blood relative, guardian or court official.
  • Where: any District or Juvenile Court in the state, regardless of where the person lives.
  • What the court must find: clear and convincing evidence that the person has an alcohol or substance use disorder and that there is a likelihood of serious harm to themselves or others because of it.
  • Legal help: the person has the right to an attorney, and the court appoints one if they cannot afford it.
  • How long: a commitment may last up to 90 days and can be shorter.

The same state guidance notes that many programs can help family and friends learn about addiction and how best to intervene. Voluntary programs are in the full list above.
